Brief Specification Description:This project is located at No. **** Yuntai Road, Pudong New Area, Shanghai, covering approximately **0 square meters adjacent to the Radiology Department in the outpatient first-floor lobby. The construction involves the new construction of two MRI machine rooms and an equipment room, along with a control room. Upon project completion and acceptance, a qualified performance shielding test report issued by a third-party authoritative inspection agency complying with national/industry standards must be submitted simultaneously. (b)Qualification requirements to be met to implement government procurement policies:This project is specifically aimed at procurement for small and medium-sized enterprises.

(c)本項(xiàng)目的特定資格要求：3.1施工資質(zhì)要求：具有住房城鄉(xiāng)建設(shè)主管部門頒發(fā)的有效的建筑工程施工總承包三級(jí)及以上資質(zhì)；
3.2具備有效的安全生產(chǎn)許可證；
3.3項(xiàng)目負(fù)責(zé)人資質(zhì)要求：擬派的項(xiàng)目負(fù)責(zé)人具有建筑工程專業(yè)二級(jí)及以上注冊(cè)建造師執(zhí)業(yè)資格，具備有效的安全生產(chǎn)考核合格證書；
3.4未被“信用中國(guó)”網(wǎng)站（******）、中國(guó)政府采購(gòu)網(wǎng)（******）、國(guó)家企業(yè)信用信息公示系統(tǒng)（******）等官方渠道列入失信被執(zhí)行人、重大稅收違法案件當(dāng)事人名單或政府采購(gòu)嚴(yán)重違法失信名單且無其他不符合《中華人民共和國(guó)政府采購(gòu)法》第二十二條的規(guī)定條件的情形。
